Clause 2 กฎกระทรวงกำหนดหลักเกณฑ์และวิธีการจดทะเบียนชื่อสกุลและค่าธรรมเนียมการออกหนังสือสำคัญ พ.ศ. 2550

ผู้ใดประสงค์จะจดทะเบียนตั้งชื่อสกุลตามมาตรา ๙ หรือตั้งชื่อสกุลใหม่ตามมาตรา ๑๗ ให้ยื่นคำขอตามแบบที่รัฐมนตรีกำหนดโดยประกาศในราชกิจจานุเบกษาพร้อมด้วยเอกสารหลักฐานตามที่ระบุไว้ในแบบคำขอดังกล่าวต่อนายทะเบียนท้องที่ ณ สำนักงานเขตที่ว่าการอำเภอ หรือที่ว่าการกิ่งอำเภอที่ตนมีชื่ออยู่ในทะเบียนบ้าน
