หมวด ๒ ใบอนุญาตและการขึ้นทะเบียน
(๑) ตาย เว้นแต่ได้ดำเนินการตามที่บัญญัติไว้ในมาตรา ๑๘ หรือสิ้นสุดความเป็นนิติบุคคล
(๒) เลิกประกอบกิจการตามมาตรา ๑๙ (๓) ถูกเพิกถอนใบอนุญาตประกอบกิจการสถานประกอบการเพื่อสุขภาพตามมาตรา ๑๕ วรรคสาม หรือมาตรา ๓๒ (๑) (๒) หรือ (๓) หรือวรรคสาม
