(๑) บัตรประจำตัวประชาชน
(๒) ใบรับรองแพทย์ตามข้อ ๕ (๒) ให้นำความในข้อ ๘ และข้อ ๙ มาใช้บังคับกับการต่ออายุใบอนุญาตตามวรรคหนึ่งด้วยโดยอนุโลม
