การยื่นคำขอรับใบอนุญาตประกอบธุรกิจหลักทรัพย์ตามข้อ ๔ ให้ยื่นต่อสำนักงานตามแบบที่สำนักงานประกาศกำหนด พร้อมทั้งเอกสารหรือหลักฐานที่กำหนดไว้ในแบบคำขอ โดยผู้ยื่นคำขอต้องชำระค่าธรรมเนียมจำนวนสามหมื่นบาท
