ผู้ที่ประสงค์จะขอให้เจ้าหน้าที่ผู้มีอำนาจรับรองสถานะการเป็นผู้มีหน้าที่รายงานตามมาตรา ๖ ให้ยื่นคำขอต่อเจ้าหน้าที่ผู้มีอำนาจตามแบบและวิธีการทางอิเล็กทรอนิกส์ที่เจ้าหน้าที่ผู้มีอำนาจกำหนด พร้อมด้วยเอกสารหรือหลักฐานตามที่กำหนดในแบบคำขอ
