การฟื้นฟูสภาพทางสังคมด้านสาธารณสุขและสุขภาพ ให้ศูนย์ฟื้นฟูสภาพทางสังคมร่วมกับกระทรวงสาธารณสุขและหน่วยงานที่เกี่ยวข้องดำเนินการติดตามและช่วยเหลือผู้เข้ารับการฟื้นฟูสภาพทางสังคมให้ได้รับการรักษาหรือรับบริการด้านสาธารณสุข โดยการติดตามและดูแลด้านสุขภาพกายและใจ รวมถึงการให้คำปรึกษา แนะนำ และให้ความรู้เกี่ยวกับการดูแลรักษาสุขภาพ
