หมวด ๑ การจดทะเบียน เครื่องหมาย และการใช้รถ
(๑) ชื่อรถ หมายเลขตัวรถ และหมายเลขเครื่องยนต์ของรถ
(๒) ความประสงค์ในการขับรถยนต์
(๓) วันเดือนปี และเวลาที่นำรถออกไปขับและกลับถึงที่
(๔) ชื่อและชื่อสกุลของผู้ขับ [คำว่า “รถยนต์” และ “เครื่องยนต์” แก้ไขเพิ่มเติมโดยมาตรา ๓ แห่งพระราชบัญญัติรถยนต์ (ฉบับที่ ๑๒) พ.ศ. ๒๕๔๖]
