(๑) วันที่บัตรหายหรือถูกทำลาย
(๒) วันที่บัตรชำรุดในสาระสำคัญ
(๓) วันที่แก้ไขชื่อตัว ชื่อสกุล หรือชื่อตัวและชื่อสกุลในทะเบียนบ้าน ผู้ถือบัตรผู้ใดย้ายที่อยู่จะขอเปลี่ยนบัตรก็ได้
