หมวด ๔ การสำรวจการประกันสังคม
(๑) แบบสำรวจ
(๒) ระยะเวลาที่เจ้าหน้าที่หรือพนักงานเจ้าหน้าที่จะส่งแบบสำรวจให้แก่นายจ้าง
(๓) กำหนดเวลาไม่น้อยกว่าสามสิบวันให้นายจ้างต้องส่งคืนแบบสำรวจที่ได้กรอกรายการแล้วแก่เจ้าหน้าที่หรือพนักงานเจ้าหน้าที่ ซึ่งต้องระบุไว้ในแบบสำรวจด้วย การประกาศตามมาตรานี้ให้ประกาศในราชกิจจานุเบกษา
