(๑) จำนวน ชนิด ราคาเสนอขายหลักทรัพย์ต่อหน่วยและมูลค่าของหลักทรัพย์ทั้งหมดที่โฆษณาชี้ชวนให้ซื้อ
(๒) ชื่อผู้เริ่มจัดตั้งบริษัทมหาชนจำกัด บริษัทหรือเจ้าของหลักทรัพย์
(๓) ประเภทของธุรกิจที่จะดำเนินการ หรือกำลังดำเนินการ
(๔) สถานที่ และวันเวลาที่จะขอรับหนังสือชี้ชวนได้
(๕) ชื่อผู้จัดจำหน่ายหลักทรัพย์ (ถ้ามี)
(๖) รายการอื่นตามที่คณะกรรมการกำกับตลาดทุนประกาศกำหนด
